Hydrogen vs. Methane SIBO: What's the Difference—and Why Does It Matter?
One of the most common things I hear from patients is, "I've been told I have SIBO, but I don't really understand what that means."
The next question is usually, "What's the difference between hydrogen and methane?"
It's a great question—and one that's incredibly important because not all SIBO is the same.
In fact, understanding which gas is being produced can often help explain why one person struggles with diarrhea while another battles years of constipation.
Let's Start with the Basics
Your digestive tract is home to trillions of microorganisms. Most of them belong in your large intestine (colon), where they help digest food, produce vitamins, and support a healthy immune system.
Your small intestine is different.
Its primary job is to digest food and absorb nutrients, so it normally contains relatively small numbers of microorganisms.
When too many microorganisms begin living in the small intestine, they start fermenting the carbohydrates you eat before your body has had a chance to absorb them. That fermentation produces gases—and those gases can create many of the symptoms people experience every day.
Hydrogen: The "Fast" Gas
Patients with hydrogen-predominant SIBO often tell me the same story.
"I eat, and within an hour I look six months pregnant."
Or...
"I can't leave the house after meals because I never know when I'll need a bathroom."
Hydrogen gas tends to speed things up.
Common symptoms include:
Bloating shortly after eating
Excessive gas
Abdominal cramping
Loose stools or diarrhea
Feeling like food "runs right through you"
Not everyone experiences every symptom, but this pattern is one I see frequently.
Methane: The "Slow" Gas
Methane behaves very differently.
In fact, methane isn't produced by bacteria at all. It's produced by organisms called methanogens, which is why you'll sometimes hear the newer medical term Intestinal Methanogen Overgrowth (IMO).
One of methane's biggest effects is that it can slow the movement of the intestines.
When I see someone who has struggled with constipation for years, feels bloated every day, and never quite feels like they've emptied their bowels completely, methane is something I often consider.
Symptoms commonly include:
Chronic constipation
Hard stools
Infrequent bowel movements
Bloating that gets worse as the day goes on
Feeling full or uncomfortable even after small meals
Many of these patients have been told for years that they "just have constipation," when there may be an underlying reason why their intestines have slowed down.
Some People Have Both
Medicine is rarely black and white.
Many patients produce both hydrogen and methane gases. They may alternate between constipation and diarrhea, experience severe bloating, or notice their symptoms change over time.
That's one reason I never believe in treating numbers on a test alone.
I treat the person sitting in front of me.
So How Do We Know Which One You Have?
A breath test can help identify whether hydrogen, methane, or both are being produced.
The results are helpful—but they don't tell the whole story.
When I evaluate someone for SIBO, I'm also asking questions like:
Why did this develop?
Is your intestinal motility working the way it should?
Are you dealing with constipation that's allowing microorganisms to linger?
Have you had food poisoning, surgery, diabetes, or another condition that's affecting your digestive tract?
Those answers are often just as important as the breath test itself.
The Bigger Picture
One of the biggest misconceptions about SIBO is that it's simply something you "kill" with antibiotics or supplements.
In reality, if we don't understand why the overgrowth happened in the first place, there's a good chance the symptoms will return.
That's why my goal isn't simply to reduce hydrogen or methane levels.
It's to understand the root cause, improve how the digestive tract functions, and help create an environment where your gut can stay healthy long after treatment is finished.

Hydrogen and methane aren't just different gases—they tell us different stories about how your digestive system is functioning.
Understanding those differences helps us create a treatment plan that's tailored to you, not just your test results.

The key is taking the time to listen, ask the right questions, and look for the underlying cause.
